Immunizations

For the health and safety of our community, all children must be up-to-date on their immunizations to physically attend classes at TBE. We will need a copy of your child’s up-to-date immunization record (click image on right to download form).

You may email the completed form to Olivia at olivia@tbetacoma.org, bring a copy of this record to the office before the school year begins, or bring a copy on the first day of in-person classes.
 

Please note: your child will not be allowed to participate in school until we have a copy of their current record on file.
